The National Youth Science Foundation works with minors in all of its programs, acting in loco parentis while student participants are in residence.  In the interest of ensuring a safe and nurturing environment for students, as well as for staff and guests of its programs, the Foundation will maintain record of a clean FBI criminal background check for each employee.

Prior to the start of student contact, staff of all Foundation programs are responsible for providing a record of a federal criminal background check.  Staff who believe they have a background check filed with the Foundation from previous programs are responsible for confirming this.

Before mailing the request to the FBI, also photocopy or scan the completed background check request materials and send a copy to the Foundation office by email, fax, or post. This lets us know that everyone will be processed before this summer!

Notes:

  • Once you have completed and filled out the forms, you will need to get a fingerprint card rolled and pressed to submit with the application–this can be done at a local law enforcement agency (does not need to be FBI; local/state police are fine).
  • Mail all forms to:  FBI, Criminal Justice Information Services Division / Attn: SCU, Mod. D-2 / 1000 Custer Hollow Road / Clarksburg, WV 26306.  Make sure you include the background check application form, the fingerprint card and the credit card payment form in your application to the FBI.  They need all these parts to complete your check.
  • Indicate NYSF as the recipient of your background check, and have the results sent to NYSF / PO Box 3387 / Charleston, WV 25333.
